Equivalent Credit Assessment Application Form

Note: please be sure to have all of your supporting documents ready to upload before submitting your Equivalent Credit Assessment Application.

Application Requirements:

  • Completed Equivalent Credit Assessment form below
  • A copy of your official transcripts from any completed years of secondary school
  • A copy of your high school diploma/certificate (if earned)
  • A copy of your government-issued identification (e.g., passport, birth certificate, etc.)
  • Assessment fee ($200 CAD). A member of our Guidance Team will contact you with directions on how to submit the fee after we have confirmed receipt of all required application documents.
Application Instructions:
  • Final grades are required for credit consideration. Progress, provisional, preliminary, and unofficial results cannot be accepted.
  • If you are submitting several documents, please name the documents appropriately (e.g., First name Last Name – Grade 10 Transcript).
  • If your transcript is not in English or French, please submit a copy of both the original transcript and a translated copy completed by a certified translator
  • Please be sure that all attached documentation is clear enough to read.
