Prerequisite Waiver Application Form

VHS requires the following documentation to process your Prerequisite Waiver Application:

Identification: A copy of your government-issued ID (e.g., passport, birth certificate, etc.)
Documentation: A copy of your official high school transcript and/or a summary of your high school courses.
- If you have completed post-secondary education, provide a copy of your transcript in addition to your high school transcript.
- If you were homeschooled, provide details on any texts that were used, course outlines, and other applicable resources to show coverage of course concepts and assessment.

Application Instructions:

Final grades are required for prerequisite waiver consideration. Progress, provisional, preliminary, and unofficial results cannot be accepted.
If you are submitting several documents, please name the documents appropriately (e.g., First name Last Name – Grade 10 Transcript).
If your transcript or report card is not in English or French, please submit a copy of both the original transcript and a translated copy completed by a certified translator
Please be sure that all attached documentation is clear enough to read.
